How Do I Prepare

You don’t need to do anything to prepare. Removing an IUD is usually less painful than putting it in.

Your doctor might suggest that you avoid sex for 7 days before your appointment. This is to prevent you from getting pregnant right after the IUD is removed if you don’t replace it with another one.

You may be more likely to get pregnant if you dont remove your IUD per the manufacturer-approved timeline, Jessica Shepherd, M.D., a minimally-invasive gynecologist at Baylor University Medical Center at Dallas, tells SELF. How long is too long to keep your IUD past its deadline?

Whats interesting about this is that some research shows IUDs are actually effective past their manufacturer-approved expiration dates, but only in certain populations . For instance, a 2014 review in the journal Contraception found that Mirena was actually effective for up to seven years when its only recommended up to five, but these results were specifically in people who were at least 25 with children at the time of insertion. In that same demographic, Paragard was effective for up to 12 years . But, on the other hand, no evidence showed that Skyla was effective past its three-year deadline in any group of people.

When Is An Iud Not A Good Option

An IUD might not be a good option for you if you have:

  • a uterus that is not the usual shape
  • a current pelvic infection.

The hormonal IUD might not be a good option for you if you have:

  • been treated for breast cancer
  • severe liver disease.

The copper IUD might not be a good option for you if you have:

  • heavy periods
  • endometriosis.

Most women have some cramping and spotting with IUDs, but this goes away within three to six months. Hormonal IUDs can reduce period cramps and make them lighter. Some of my patients periods went away altogether. Copper IUDs can make periods heavier and cramps worse, but this usually goes away over time.

How We Handle Pregnancy With An Iud

Our first step is to determine the type of pregnancy, which could be either:

Intrauterine, which is the normal location for pregnancy development, allowing for fetal growth over 9 months, or

Ectopic, which is when a fertilized egg implants in a fallopian tube or other location outside the uterus and poses serious health risks to the woman

We used to believe that having an IUD increased the risk of having an ectopic pregnancy. Now we understand that because IUDs are so effective at preventing intrauterine pregnancies that, if a pregnancy does occur, theres a higher likelihood that it developed outside the uterus. But patients arent at increased risk for ectopic pregnancies just because they have an IUD.

Next, we look for the IUD strings in the cervix to potentially remove the device. Studies have shown that the risk of miscarriage, preterm birth, and infection of the amniotic sac and fluid before delivery is significantly higher when IUDs are left in place during pregnancy compared to pregnancies in which the IUD is removed. However, while taking out the IUD improves the pregnancy outcome, there is still a higher risk of pregnancy complications compared to patients who never had an IUD in place. For most women whose IUD strings are visible through the cervix, we recommend removal of the IUD at the first visit.

Getting Pregnant After Removing An Iud

Although getting pregnant with an IUD is rare, it’s easier to conceive after you remove the device.

If you have decided to try to conceive, give your OB-GYN a call to have your IUD removed, and get busy trying. Some doctors recommend waiting three months to give your body time to bounce back to its usual menstrual cycle. This will help you get a sense of ovulation and fertility cycles. But according to Dr. Perez, there’s no medical need to wait: “You can ovulate and get pregnant the next month!”

Effectiveness For Birth Control

In clinical studies, Kyleena was effective in preventing pregnancy for up to 5 years.

During the studies, adult females used Kyleena for 3 to 5 years. In each year of the studies, for every 100 women who used Kyleena, the following pregnancy rates were reported:

  • 0.16 pregnancies in the first year
  • 0.38 pregnancies in the second year
  • 0.45 pregnancies in the third year
  • 0.15 pregnancies in the fourth year
  • 0.37 pregnancies in the fifth year
  • 1.45 pregnancies over all 5 years of use

Overall, Kyleena prevented pregnancy for 5 years in 98.6% of women who used it. The pregnancy rate for women who used other birth control drugs in these studies isnt known.

Changes To Your Period

During the first 3-6 months after insertion of your Kyleena IUD, you may have a higher chance of experiencing bleeding and spotting. Your period may become irregular, and/or your periods may be heavier or longer than usual.

Although breakthrough bleeding and spotting will most likely decrease within the first three months after insertion, your periods may continue to be irregular.

Over time, your periods will likely become shorter and lighter. Because the progestin in Kyleena thins your uterine lining, bleeding may decrease the longer your Kyleena IUD has been in place.

Your periods may stop altogether. About 12% of women stop having periods after using Kyleena for one year.

The Chances Of Getting Pregnant With An Iud Are Small

Your chance of getting pregnant with an IUD is 0.1 to 0.2 percent. That means, for every 1,000 women with an IUD, one to two of them will become pregnant, says Thomas Ruiz, MD, Lead OB-GYN at MemorialCare Orange Coast Medical Center.

Most of the time, if you get pregnant with an IUD it’s because of one of the following three factors:

  • The IUD is not properly placed in the uterus.
  • The IUD is properly places, but your uterus contracts and dislodges the device, which you’ll probably notice either because the IUD will fall out or the IUD strings will feel out of place. That’s why a follow-up appointment to check placement is important, Ruiz says.
  • A woman has a uterine abnormality that neither she nor the doctor knows about until it’s too late.

But even with proper placement and no abnormalities, the IUD, like any other form of birth control, does not completely guarantee pregnancy prevention. “We don’t always know why it happens, but it can fail,” Ruiz says.

These IUDs are also reversible contraception, meaning that when the IUD is removed, you can get pregnant. Most patients are able to get pregnant within 12 months after IUD removal. Only your healthcare provider can determine which medication is better for you, taking into account your medical history.

How An Iud Prevents Pregnancy

There are two types of IUDs one that contains copper and one that produces progestin, which is a synthetic form of the hormone progesterone. Both create a hostile environment for sperm to prevent it from fertilizing an egg.

What Happens During Iud Removal

The procedure is the same for any form of IUD — copper or hormonal .

You will lie on a table with your knees bent and your legs apart. Your doctor will insert a special tool called a speculum into your vagina to widen the opening.

Threads from your IUD should hang out of your cervix into your vagina. Using a special grasping tool, your doctor will gently pull on the strings and pull out the device. An IUD is T-shaped, and its arms will fold up as it slides out.

If the doctor can’t see or reach the threads, they can use a special hook or other tool to pull them into view.

  • There are also risks if you get pregnant while using Kyleena and the pregnancy is in the uterus. Severe infection, miscarriage, premature delivery, and even death can occur with pregnancies that continue with an intrauterine device . Because of this, your healthcare professional may try to remove Kyleena, even though removing it may cause a miscarriage. If Kyleena cannot be removed, talk with your healthcare professional about the benefits and risks of continuing the pregnancy and possible effects of the hormone on your unborn baby.

  • If you continue your pregnancy, see your healthcare professional regularly. Call your healthcare professional right away if you get flu-like symptoms, fever, chills, cramping, pain, bleeding, vaginal discharge, or fluid leaking from your vagina. These may be signs of infection.

  • at least 6 weeks after an abortion or miscarriage that occurred during the third to the sixth month of pregnancy
  • at least 6 weeks after childbirth

Before placing Kyleena, your healthcare provider may have you take a pregnancy test. This way, they can make sure youre not pregnant before inserting Kyleena into your uterus.

Talk with your doctor about the best time for you to start using Kyleena.

* Your menstrual cycle begins on the first day of a period. In other words, day 1 of your menstrual cycle is the first day you have period bleeding.

Ive Been Told Iuds Are Unsafe Is This True

Risks are small but I do like patients to know what to expect. Irregular cramping and bleeding are common, but usually resolve in four to eight weeks. More rare complications include malposition or perforation. Malposition means the IUD has an abnormal position in the uterus. Perforation is when the IUD passes through the wall of the uterus in the abdominal cavity. Care with placement helps avoid these issues. If there are concerns, an ultrasound can help reassure us of the location.

IUDs today are better and safer today after doctors and manufacturers learned a few lessons learned early on. Back in the 1970s, an IUD called Dalkon Shield was linked to pelvic infection that caused infertility and death. That IUD had multiple flaws, including a design that allowed bacteria to get into the uterus and made it difficult to insert. This caused failures and pregnancies.

Most people can get an IUD safely. But you shouldnt get an IUD if you:

  • Have or might have an STD or other infection
  • Think you might be pregnant
  • Have cervical cancer that hasn’t been treated
  • Have cancer of the uterus
  • Have vaginal bleeding thats not your period
  • Have an allergy to copper, Wilsons Disease or a bleeding disorder

Overall, IUDs are a great form of birth control. They are highly effective because they limit the chance of you making a mistake. You dont need to remember to swallow a pill and you cant use it incorrectly.

Do You Ovulate With Kyleena Iud

Yes, you still continue to ovulate while using Kyleena. How come you still ovulate while on Kyleena? Well, Kyleena prevents pregnancy by thickening the cervical mucus that avoids penetration of sperms. It also makes the uterine environment unfavorable for sperm survival. So, as it does not affect your ovulation, you do ovulate with Kyleena IUD in place. Many women who use a hormonal IUD will continue to ovulate, however, their cycle may be disrupted depending on the hormonal dosage of the IUD inserted. This is not very common with Kyleena.
