If your child wears contact lenses, your doctor or eye doctor may recommend that the lenses not be worn until the infection is gone. Then, disinfect the lenses and their storage case at least twice before letting your child wear them again. History Of Present Illness

A 19-year-old pregnant woman at 19 weeks gestation was referred to the University of Iowa Hospitals and Clinics from an outside ophthalmologist for evaluation and treatment of suspected Acanthamoeba keratitis.

The patient had been wearing daily disposable soft contact lenses for 3 weeks continuously followed by 1 week of removal at nighttime prior to her presentation. She began to notice left eye pain and decreased vision 1 week before presenting to an optometrist, who initially prescribed a combination antibiotic and steroid drop and subsequently a steroid-only drop. These therapies did not improve her symptoms. She was referred to an outside ophthalmologist who suspected Acanthamoeba keratitis and referred her to UIHC.

At presentation to our clinic, she had severe left eye pain, redness, and photophobia. She had been using preservative-free artificial tears 2 or 3 times per day with no symptomatic relief. She denied swimming or using a hot tub in the previous 2 months, but had showered with contact lenses in place on multiple occasions. At the time of presentation, she had no established prenatal care.

Intra Ocular Pressure Modifying Changes

The IOP is known to decline in pregnancy to the tune of 10%, with the peak decline in the 12th to 18th week in the ocular hypertensive group. This drop may last for several months post-partum period. The pregnant ladies also have a reduced diurnal fluctuation in their IOP as compared to their pre-pregnancy diurnal variation.

This drop in IOP in pregnancy is a result of an increased outflow facility, caused by an increased uveo-scleral outflow and a decrease in the episcleral venous pressure consequent to the decreased venous pressure in the upper part of the body. While, pregnancy induced acidosis adds to this IOP fall, change in ocular rigidity is not a factor in this IOP fall as the measurements by indentation and applanation have been comparable. Thus the pre-existing glaucoma tends to improve during the pregnancy. Hørven and Halvard found a moderate decrease in intra ocular pressure during both the second part of pregnancy and the first two months after delivery.

Conjunctivitis Or Pink Eye

Conjunctivitis refers to a condition in which conjunctiva, a thin membrane that shapes the eyelid and shelters the front part of the eye gets inflamed. The eye may look bloodshot as blood vessels on the eyes surface dilate or widen, forcing additional blood into the eye. Usually, the pink eye has other symptoms too like pain in the eyes, irritation, burning or itching, swelling of the infected eye, and watery discharge. Conjunctivitis in pregnancy may get triggered by an allergy, infection , stress on the eyes, plugged tear duct, or chronic dry eyes. Conjunctivitis is a contagious condition and thus should be treated promptly. Your eye doctor, on evaluation, may determine the cause of your conjunctivitis and then recommend antibiotics, if necessary. Alternatively applying a cool washcloth on the affected eye may provide some relief from the irritation.

What Is Pupil Dilation

The pupil is the circular hole at the very center of your eye, surrounded by the colored area known as the iris. The iris is made up of small muscle fibers that control the size of the pupil, thereby controlling the amount of light allowed to reach the retina, the sensory area at the back of the eye. Normal pupils get smaller, or constrict, with bright light levels, and enlarge, or dilate, with low or dim light. Changes in pupil size also occur with focusing your eyes and in response to emotion.

Pupil dilation can also result from a variety medications or drugs. Eye care professionals often take advantage of eye drops that cause pupil dilation so they can get a better look at the back of the eye during an eye exam. In most other circumstances, pupil dilation from medications or drugs is considered a side effect. Medications and drugs that can cause pupil dilation include and cold medications, decongestants, amphetamines, cocaine, lysergic acid diethylamide , and marijuana.

Poisonings can also cause pupil dilation. Pupil dilation that results from poisonings, medications, and drug use typically affects both pupils equally, unless it is from eye drops that are used only in one eye. It is generally temporary.

Some neurologic conditions, such as , tumor, or brain injury, can also lead to pupil dilation, which can occur in one or both eyes. Pupils that do not respond to light or other stimuli are called fixed pupils. Often, abnormally are also fixed pupils.
