Even women with the most consistent menstrual cycles can suddenly skip their period, which most likely means they didnt ovulate. The reason is usually something simple like experiencing higher than normal stress, but a missed period could also have a medical reason. The most obvious medical reason is pregnancy, and testing for it is quick and easy. Women who recently had babies and are still breastfeeding also dont usually ovulate although they could, so birth control is still important.

If you are nearing 50 years of age, you could be experiencing perimenopause symptoms, which include irregular ovulation and periods. Certain types of birth control, such as intrauterine devices , can also affect ovulation and periods, as can low body weight, particularly low body weight related to an eating disorder like anorexia nervosa. Serious medical conditions are less likely but possible and could include polycystic ovary syndrome, pelvic inflammatory disease and premature ovarian failure.

What If You Have Polycystic Ovarian Syndrome

Polycystic ovarian syndrome , a hormonal disorder, is one of the most common reasons for female infertility. It affects between 6% and 12% of American women of reproductive age.

There is no single test to identify PCOS, but a doctor will determine if a woman fulfills two of the following three criteria,the CDC said:

  • Irregular periods or no periods, caused from lack of ovulation
  • Higher than normal levels of male hormones that may result in excess hair on the face and body, acne or thinning scalp hair
  • Multiple small cysts on the ovaries

It is not clear why some women develop this syndrome, although it is often diagnosed when they struggle to get pregnant.

There is a strong link between PCOS-related infertility and weight. About 40% to 60% of women with PCOS are overweight or obese, and healthy eating and exercise have been shown to improve reproductive problems in women with PCOS, according to a 2019 article in the journal Therapeutic Advcances in Reproductive Health. A number of prescribed medications may also induce ovulation and regulate insulin levels, such asletrozole and metformin. High levels of insulin push thepituitary gland to release large amounts of hormones thatdisrupt ovulation.

Doctors recommend in-vitro fertilization if other interventions have not worked, or as a primary treatment if monitoring, cost and accessibility are not issues.

Signs You Are Ovulating

Counting off days to estimate ovulation is a process that works well for women who have regular menstrual cycles, but the process is much more challenging if their periods are irregular. For women who cant rely on predictability in their monthly cycles, the frustration begins with trying to pinpoint when they will have their period and ends with calculating when they are likely to ovulate all while knowing the estimates could end up being meaningless if their period falls at a dramatically different time than the previous month.

Fortunately, other physical signs of ovulation are often present to help you pinpoint when your body is preparing to release an egg. Vaginal discharge or mucus around this time is thinner, clearer and more slippery to help sperm move more efficiently through the reproductive tract. You may experience cramping on one side around the time you ovulate, but this is not true for every woman.

Your basal temperature body temperature when you first wake up but before you get out of bed and start moving around may be slightly elevated around the time of ovulation. You may need to track this reading for a few months to confirm your pattern. Over-the-counter ovulation predictors have also helped many women determine their estimated ovulation dates. If youre working with a doctor, he or she may test for LH levels, which start climbing about 36 hours before ovulation and peak at about 12 hours before.

When Should I See A Doctor For A Clinical Pregnancy Test

Most women who have a positive pregnancy test should wait a week or two before calling the Ob/Gyn office for a pregnancy blood test. We suggest waiting because the rate of early pregnancy loss is high, and its possible that it might have been a chemical pregnancy.

However, women with a history of ectopic pregnancy or current abdominal pain should call the doctor right away to be examined. Ovulation usually happens about 12 to 16 days before the start of your next period, so it helps to know your cycle length before you start trying to conceive.

You may never have thought about when you might ovulate within your cycle, and if you have been using a hormone contraceptive such as the pill, you wont have had a natural menstrual cycle for a while.

As a first step, mark on your diary the dates that you bleed during a period. You can then count how many days from the first day of your period to your next period to work out the length of your cycle.

If you want to work out when you ovulate, there are a number of things you can try, such as monitoring your cervical mucus changes and body temperature or using ovulation test kits.

So When Is Pregnancy Most Likely

The only time you can get pregnant is during your fertile window.

An egg only lives for about 24 hours after being released from your ovary, and sperm can only live for up to five days inside the body.

That means you can only get pregnant if you have sex:

  • in the four to five days leading up to ovulation
  • on the day of ovulation
  • on the day after ovulation

If youre looking to conceive, the best time to have sex is right before ovulation. This will give sperm time to reach the fallopian tube and meet the egg there.

After that, if no sperm has fertilized the egg, it will dissolve. You wont be able to get pregnant until your cycle restarts.

Exceptions: When You Can Get Pregnant Right Before Your Period

1.If what you think is a period isnt actually a period. Its possible to mistake your period for ovulation bleeding, especially if it comes with period-like symptoms. Should you mistake an ovulation bleed for a period, right before your period overlaps with your fertile window, and the likelihood of getting pregnant is quite high.

2.If you have a long fertile window. Ovulation typically occurs around day 14 of a 28-day menstrual cycle, but everyones cycle is different. One study found that most women reach their fertile window earlier and others much later than day 14.

Its rare, but you may have a uniquely long, or uniquely late-arriving fertile window. This can occur naturally, or be a result of stress, medications, thyroid disorders, or other conditions. No matter when it occurs, ovulation always presents the possibility of getting pregnant, even if very late in your cycle.

Just note that after fertilization, the egg still needs to be implanted into the uterine wall. That can take a week or more. If you fertilize an egg during a fertile window that is very close to your period, the odds of a failed implantation increase because the uterine lining is shed during your period, potentially shedding the fertilized egg along with it.
