Fillers And Botox: Safe During And After Pregnancy
In my last post, I discussed why it is never wise to have cosmetic surgery while pregnant. Today, Ill talk about the safety of fillers and Botox during and immediately following pregnancy.
Injectable filling material such as Juvéderm, Restylane, Radiance, and Artefill typically do not enter the circulation and therefore it would not appear to be dangerous during pregnancy.
Understand that the fillers and the wrinkle-softeners, Botox and Dysport, are all degraded or metabolized by the body and their end products would not be expected to be injurious any more than any other medication that the mother may be taking.
However, I am just against any non-essential service being rendered to a patient during pregnancy. In medicine, we dont have 100% certainty as to all the interactions between drugs and the body. So, why take the chance?
After a baby is delivered, the question often comes up about whether or not it is safe to have fillers, Botox or Dysport, and whether or not these medications might enter the breast milk. Again, my response is one of being extremely conservative: Not likely to cause a problem, but why take a chance?
Do you wait until you are done breastfeeding or take a hiatus in the breastfeeding until the injections are given? Yes, both are good options.
Patients Who Are Lactating
There are very few studies and reports on the safety of cosmetic procedures in patients who are lactating. The main concern of pursuing any modality in patients who are lactating is the systemic absorption of agents and subsequent incorporation into breast milk, which can affect neonatal growth and development. Lee et al. summarized that most cosmetic procedures such as botulinum toxin A, chemical peeling, and lasers are safe to use during lactation since there is low concern for significant systemic absorption of any of the agents used in these procedures. Procedures which necessitate the redistribution or removal of fat such as fat transfer or tumescent liposuction are not recommended. Sclerotherapy should also be avoided during lactation . Generally, hypertonic saline solutions that are used in sclerotherapy are safe but there are no data on whether other sclerosing solutions are excreted in breast milk, which has led to the recommendation to avoid this therapy while breastfeeding. However, anecdotally, some women have pursued this therapy and continued breastfeeding with pumping and discarding the breast milk in the first 48 hours after treatment. Nevertheless, there are no reports on the outcomes and/or complications or success rates of this methodology. Therefore, the safety of this procedure during lactation cannot be meaningfully commented on at this time.
