In the 1970s, the average age of first-time mothers in the United States was 21. By 2020, that average age had jumped to 26. Studies show this trend is mostly due to career goals and financial concerns children arent cheap! Unfortunately, evolution has not caught up with the newer preference of becoming pregnant later in life, and our bodies face some unique pregnancy challenges as we age.
As OB/GYNs, weve seen the number of patients asking us questions about pregnancy over 40 rise as well. We thought we would address a few of the most common questions we get and offer some advice for those who are trying to conceive later in their reproductive lifespan.
What Are My Chances Of Pregnancy After 40
It’s possible, but almost half of women over 40 experience fertility issues, according to the CDC. Women over 40 have about a 5 percent chance of getting pregnant in any single ovulation cycle.
At 40, your chances of getting pregnant within a year are about 40 to 50 percent. To compare, a woman in her mid-30s has about a 75 percent chance. And by age 43, a woman’s chance of getting pregnant within a year drops significantly to only 1 or 2 percent.
Why the steep drop-off? It’s all about the eggs. When you reach puberty, your eggs number between 300,000 and 500,000, and after that you lose about 13,000 of them a year. Over the years, this steady drop in egg supply leaves you with about 25,000 eggs by age 37 the age that also coincides with a precipitous drop in fertility. By age 43, your egg supply is near its end. Your risk of pregnancy complications, such as high blood pressure and gestational diabetes, increases after 35 as well, and continues to rise into your 40s.
Miscarriage rates begin to skyrocket in your 40s as well. At 40, the average chance of a miscarriage is about 40 percent, and that rises to around 80 percent by age 45. The same underlying mechanism that causes miscarriage also increases the risk of Down syndrome: At 40, your chance of conceiving a child with Down syndrome is one in 100 at 45, it’s one in 30.

How Does Age Affect Labor And Delivery
Vaginal delivery may be less likely after the age of 40. This is primarily due to fertility treatments that can increase risk for premature birth. You may also be at an increased risk of preeclampsia, which may necessitate a cesarean delivery to save both mother and baby.
If your baby is delivered vaginally, the process may be more challenging as you get older. Theres also an increased risk of stillbirth.
Many women do successfully deliver healthy babies at or over the age of 40. Talk to your doctor about what to expect, and come up with a backup plan. For example, if youre planning a vaginal delivery, talk to your partner and support group about what help youll need if you require a cesarean delivery instead.
Age in and of itself does not increase your risk for multiples. However, women who use fertility drugs or IVF for conception are at a higher risk of twins or multiples. Infertility. . This is because of the way the medications increase ovulation.
Having twins also increases the risk that your babies will be more prematurely.

Yes, it’s possible to get pregnant at 45, though conceiving naturally is unlikely. A woman’s prime fertility time is between her late teens and her 20s, and once you reach your mid-30s, your ability to get pregnant starts to decline.

How To Conceive After 40
The process of conceiving after 40 is no different for many people. For heterosexual couples to conceive, the couple must have intercourse during the womans fertile window. This window includes the days leading up to ovulation and the day of ovulation.
Using ovulation predictor tests can help pinpoint the release of eggs. Monitoring basal body temperature can help a person check whether they have ovulated. Doing this is important as a woman ages, as some women stop ovulating or do not ovulate as frequently.
To monitor basal body temperature, people can take their temperature at the same time each morning as soon as they wake up. It is important to do this immediately in the morning, after at least 34 hours of sleep, and using a digital thermometer.
After ovulation, the average temperature rises slightly by 0.51°F and remains elevated until a woman gets her period. The temperature then returns to baseline.
Basal body temperature is not always an accurate measure of ovulation. Several factors, such as stress and alcohol consumption, can affect the measurements.
Women who are single or in a same-sex relationship may choose fertility treatments such as IUI or IVF. These treatments are also an option if one person in a relationship has infertility.
If either partner has any preexisting conditions, such as diabetes or polycystic ovary syndrome , they should see a doctor for testing before trying to get pregnant.

The decision to conceive, carry, deliver and nurture a human being well into adulthood has serious consequences at any age, but at 43 additional challenges are present before and after conception.

Age And Fertility: Getting Pregnant At An Older Age
So what does age got to do with it? In a word, everything.
We know that while men continuously produce new, healthy sperm into their later years , women didnt get so lucky in the reproductive lottery.
Thats because a woman is born with all the eggs shell ever have in life about 2 million and she loses eggs all the time. Not only does she lose eggs in the years between birth and the start of her period and during pregnancy and breastfeeding, but she also loses eggs every month as part of her menstrual cycle.
That means that from those 2 million eggs at birth, a woman really only ovulates about 500 eggs during her reproductive lifetime, losing the rest. And just as her egg supply shrinks with time, so does the quality of her remaining eggs. Like the rest of our bodies, eggs cannot escape time-related cellular damage, which results in abnormal eggs that either fail to implant in the uterus, lead to miscarriage or result in an affected baby with a chromosomal condition.
This gradual fertility decline ultimately results in menopause, when a womans egg supply is in the hundreds and she no longer has a regular menstrual cycle.
But, as mentioned above, the process does not happen overnight. Its also not an all or nothing mechanism though the rates of age-related infertility, miscarriage and aneuploidy go up with a womans age, young women can experience these things too, just in smaller numbers.
Who’s Attempting Conception At 43
The term used by obstetricians for women over age 35 who are conceiving for the first time is elderly primipara, yet the 43-year-old in 2010 may feel anything but elderly. She has likely completed some college, if not one or more degrees, built a professional career and either married late in life or chose not to marry, according to the Pew Research Council.
