Changes To Your Period
During the first 3-6 months after insertion of your Kyleena IUD, you may have a higher chance of experiencing bleeding and spotting. Your period may become irregular, and/or your periods may be heavier or longer than usual.
Although breakthrough bleeding and spotting will most likely decrease within the first three months after insertion, your periods may continue to be irregular.
Over time, your periods will likely become shorter and lighter. Because the progestin in Kyleena thins your uterine lining, bleeding may decrease the longer your Kyleena IUD has been in place.
Your periods may stop altogether. About 12% of women stop having periods after using Kyleena for one year.

Can You Get Pregnant With An Iud
Yes, You can pregnant with an IUD. It is possible to pregnant with an IUD. You can get pregnant while using an IUD, but the chances are very small. Less than 1% of women with copper or hormonal IUDs become pregnant each year.
An IUD must remain in your uterus to prevent pregnancy. But sometimes it can move out of place and slip into your cervix, which is the bottom of your uterus. If this happens, you are more likely to become pregnant.
If you become pregnant while using an IUD, your health and that of your baby may be at risk. Tell your doctor if you think your IUD has failed so they can keep you safe.
Types of Pregnancy With an IUDIf your IUD fails, your doctor will find out what type of pregnancy you have:
Intrauterine pregnancy: This is a normal pregnancy in your uterus where your baby will grow for 9 months.Ectopic pregnancy: The embryo tries to grow outside your uterus.
This usually happens in your fallopian tubes, which carry eggs to your ovaries and then to your uterus.
You can also get an ectopic pregnancy in your ovaries, abdomen or cervix. The pregnancy may not progress normally in one of these areas.
The doctor will terminate the ectopic pregnancy to save you from possible bleeding that could put your life at risk.
Because the IUD prevents pregnancy in your uterus, you are more likely to have an ectopic pregnancy than a regular pregnancy.
But that doesnt mean youre at higher risk for ectopic pregnancies in general just because you have an IUD.
Pregnancy & Intrauterine Devices
While extremely rare, pregnancy can occur with an IUD. If you find yourself in this scenario, it is imperative that you talk to your OG-GYN immediately. Unfortunately, when you get a positive result on a home pregnancy test with an IUD in place, a large percentage of the time the pregnancy is extrauterine. More commonly referred to as ectopic pregnancy, this is a life-threatening condition that needs to be addressed immediately.
That is not to say that all pregnancies will turn out this way, but it is always best to err on the side of caution since the risk of ectopic pregnancy is higher in women who use an IUD as their main form of contraception. Your doctor will conduct an ultrasound and an exam to determine if the pregnancy is viable or not.
If they determine that the implantation occurred effectively, you can go on to have a healthy pregnancy. In this instance, it is recommended that the IUD is removed immediately. This will allow you to achieve an optimal outcome for you and your baby.
Research shows that women who conceived with an IUD in place and chose to continue the pregnancy without removing the IUD need close follow-up, as there appears to be a higher risk of adverse pregnancy and neonatal outcome. This is especially true when the IUD has moved out of place and into the cervix. This position can increase your risk of miscarriage.

Kyleena And Emergency Contraception
Kyleena shouldnt be used as emergency contraception. And you dont need to use emergency contraception along with Kyleena.
Emergency contraception is used to prevent pregnancy after youve had sex without birth control. But if youre using Kyleena, youre protected from becoming pregnant during sex.
An example of emergency contraception is Plan B, which is also called the morning after pill.

Why You Shouldn’t Get An Iud
If you get pregnant with an IUD in place, there’s an increased risk of ectopic pregnancy and other serious health problems. It’s possible to get an infection if bacteria gets into your uterus when the IUD is put in. If the infection isn’t treated, it may make it harder for you to get pregnant in the future.
Ive Been Told Iuds Are Unsafe Is This True
Risks are small but I do like patients to know what to expect. Irregular cramping and bleeding are common, but usually resolve in four to eight weeks. More rare complications include malposition or perforation. Malposition means the IUD has an abnormal position in the uterus. Perforation is when the IUD passes through the wall of the uterus in the abdominal cavity. Care with placement helps avoid these issues. If there are concerns, an ultrasound can help reassure us of the location.
IUDs today are better and safer today after doctors and manufacturers learned a few lessons learned early on. Back in the 1970s, an IUD called Dalkon Shield was linked to pelvic infection that caused infertility and death. That IUD had multiple flaws, including a design that allowed bacteria to get into the uterus and made it difficult to insert. This caused failures and pregnancies.
Most people can get an IUD safely. But you shouldnt get an IUD if you:
- Have or might have an STD or other infection
- Think you might be pregnant
- Have cervical cancer that hasnt been treated
- Have cancer of the uterus
- Have vaginal bleeding thats not your period
- Have an allergy to copper, Wilsons Disease or a bleeding disorder
Overall, IUDs are a great form of birth control. They are highly effective because they limit the chance of you making a mistake. You dont need to remember to swallow a pill and you cant use it incorrectly.

Before Taking This Medicine
Kyleena can increase your risk of developing a serious pelvic infection, which may threaten your life or your future ability to have children. Ask your doctor about this risk.
Do not use Kyleena during pregnancy. If left in place during pregnancy, the IUD could cause severe infection, miscarriage, premature birth, or death of the mother. The hormone in Kyleena may also cause unwanted effects in a female newborn.
Tell your doctor right away if you become pregnant. If you continue the pregnancy, watch for signs such as fever, chills, cramps, vaginal bleeding or discharge.
You should not use Kyleena if you are allergic to levonorgestrel, silicone, silica, silver, barium, iron oxide, or polyethylene, or if you have:

abnormal vaginal bleeding that has not been checked by a doctor

an untreated or uncontrolled pelvic infection

endometriosis or a serious pelvic infection following a pregnancy or abortion in the past 3 months

pelvic inflammatory disease , unless you had a normal pregnancy after the infection was treated and cleared

past or present cancer of the breast, cervix, or uterus

Effectiveness For Birth Control
In clinical studies, Kyleena was effective in preventing pregnancy for up to 5 years.
During the studies, adult females used Kyleena for 3 to 5 years. In each year of the studies, for every 100 women who used Kyleena, the following pregnancy rates were reported:
- 0.16 pregnancies in the first year
- 0.38 pregnancies in the second year
- 0.45 pregnancies in the third year
- 0.15 pregnancies in the fourth year
- 0.37 pregnancies in the fifth year
- 1.45 pregnancies over all 5 years of use
Overall, Kyleena prevented pregnancy for 5 years in 98.6% of women who used it. The pregnancy rate for women who used other birth control drugs in these studies isnt known.

When Was It Approved
Pretty recently, in fact! It was approved by the FDA on September 19, 2016. But that doesnt mean anyone can just run out to the doctor and get one inserted right away. Bayer, the manufacturer of the IUD, announced on the 19th of September that the device had just been approved.
Until this point, there were only four IUDs on the market. The main two that everyone knows about are Paragard, a copper based IUD, and Mirena, a hormonal based IUD. Two lesser known IUDs that are out there are Liletta and Skyla, which are both hormonal based IUDs.
Liletta and Skyla last for 3 years each, while Mirena lasts for 5 years, and Paragard lasts for 10 years. Having another option out there is a great thing for women who are looking into their birth control options. There are so many options for the birth control pill, there are options for the shot, and there are even options for ring-shaped devices that sit in the vaginal cavity. Its about time that more options for the IUD market became available.

How We Handle Pregnancy With An Iud
Our first step is to determine the type of pregnancy, which could be either:
Intrauterine, which is the normal location for pregnancy development, allowing for fetal growth over 9 months, or
Ectopic, which is when a fertilized egg implants in a fallopian tube or other location outside the uterus and poses serious health risks to the woman
We used to believe that having an IUD increased the risk of having an ectopic pregnancy. Now we understand that because IUDs are so effective at preventing intrauterine pregnancies that, if a pregnancy does occur, theres a higher likelihood that it developed outside the uterus. But patients arent at increased risk for ectopic pregnancies just because they have an IUD.
Next, we look for the IUD strings in the cervix to potentially remove the device. Studies have shown that the risk of miscarriage, preterm birth, and infection of the amniotic sac and fluid before delivery is significantly higher when IUDs are left in place during pregnancy compared to pregnancies in which the IUD is removed. However, while taking out the IUD improves the pregnancy outcome, there is still a higher risk of pregnancy complications compared to patients who never had an IUD in place. For most women whose IUD strings are visible through the cervix, we recommend removal of the IUD at the first visit.
How Long Does It Last
Unlike birth control pills, which you have to take daily, Kyleena is good for about 5 years, which is when the amount of hormones in the device drops to a level that makes pregnancy possible again. If pregnancy is not in the future plans, then Kyleena needs to be removed after the 5th year, and replaced, or another birth control method needs to be used after removal.
It can be replaced with a new Kyleena device if continued contraceptive protection is wanted.
Kyleena, due to how it needs to be inserted and all that jazz, does need a prescription and a doctors visit to get, so unfortunately, its not something like condoms where all it takes is a quick trip to the pharmacy.
However, once its in, its in for the 5 years, unless something happens and it comes loose, which most of the time will happen before that follow up visit and will be easily spotted by the doctor.

How Do I Prepare
You dont need to do anything to prepare. Removing an IUD is usually less painful than putting it in.
Your doctor might suggest that you avoid sex for 7 days before your appointment. This is to prevent you from getting pregnant right after the IUD is removed if you dont replace it with another one.
Taking A Pregnancy Test
If you suspect youre pregnant, consider taking an at-home pregnancy test. These tests claim to be up to 99% effective. In most cases, the tests provide accurate results, but their reliability depends on following the instructions and not testing too early after conception.
Its also recommended to discuss your concerns and symptoms with your healthcare provider to ensure the IUD is working correctly. They may also perform a pregnancy test in the office to confirm whether you are pregnant.

Infertility Is Caused By Scarring
Infertility is not caused by the IUD or the removal process. Instead, infertility is caused by disease or infections of the organs and tissues in your pelvic region and the scarring that if often leaves behind. Absent those conditions, your ability to get pregnant after using an IUD is the same as it was before your IUD.
Notwithstanding your immediate fertility, some doctors recommend their patients wait for a cycle or two after IUD removal before trying to conceive. By waiting, it is easier to track your cycle and determine the exact date of conception.

Will My Periods Change
With hormonal IUDs, many women have fewer cramps. For the first few months, some women have irregular spotting. Eventually, most women have light periods or no period at all. Pregnancies rarely happen with IUDs, but if not having a period will make you constantly worry that youâre pregnant, you may want to consider the copper IUD instead.

This month!! Thats right, Kyleena is available this October 2016! That being said, some doctors may take longer to to get a hold of Kyleena for their patients, so make sure to ask. This also leaves more time to ask about Kyleena and find out if its the right choice. And sometimes, it gives us the chance to save up the money to get the IUD.
Pricing has yet to be released for Kyleena, but the medical exam and insertion alone can cost upwards of $500 to $900 according to Planned Parenthood. That having been said, insurance may cover it, so check with them to find out what theyll cover.
Kyleena may be the right choice if lower hormone dosages are whats needed. IUDs arent for everyone, but with this lower dose option, I can see the market growing even more and revolutionizing more so than it has thus far, and thats saying something!
