If there is a significant change in your symptoms then the nurse will make an appointment to come to EPAU and be either rescanned or to be assessed by the doctor. If bleeding unmanageable then the nurse may advise to be assessed on ward 30 or the Emergency department depending on your symptoms.

  • For a while, take some time off work to relax and get off your feet
  • Your baby may require medicine to protect him from Rh disease. Rh disease occurs when your blood and that of your baby are not compatible . This can lead to serious health problems, even death, for your baby.
  • Do not sex, use tampons or have sex with others
  • You may need surgery or hospitalization if you have severe bleeding.

What Are The Causes Of Abdominal And Back Pain

Some of the aches and pains experienced during pregnancy are thought to be due to hormonal changes. Large amounts of the hormone progesterone are produced, which are needed to sustain pregnancy. In addition to this, progesterone acts on the muscles, ligaments and joints causing them to become slacker and more flexible. This hormonal effect is thought to be responsible for some of the stitch like pains that some women experience in the lower part of the tummy and in some cases this can be quite severe. The same hormones can be responsible for constipation during pregnancy this can also cause abdominal pain.

The enlarging womb is made up of layers of muscles and is held in place in the pelvis by supporting ligaments. As the womb expands to accommodate the developing baby, it can pull on the ligaments and muscles, to cause these growing pains.

The backache that some women get in early pregnancy is also thought to be due to a hormonal effect, and the supporting muscles are softer.

Abdominal and/or backache can also be a sign of a urine infection. This may also cause burning or stinging when passing urine and the need to pass urine more frequently. You must inform your doctor of these symptoms, so that a urine test can be obtained and treatment given if needed.

When Should You Contact Your Doctor

Its always a good idea to reach out to your doctor if youre experiencing spotting during your pregnancyor anything else that seems out of the ordinary.

Dont be worried about bothering them or asking too many questions. According to Dr. Ward, its vital to always give your doctor a full description of your symptoms its what helps your healthcare provider determine if you need further evaluation.

Additional symptoms, such as cramping or fever, almost always warrant a call, even if the spotting is mild, Ward says. Pain or heavy bleeding that requires a pad requires immediate attention, as does any bleeding that occurs in the second or third trimesters, as these may be signs of a bigger problem.

With all that, try to remember that spotting during pregnancy is common, and hopefully knowing what to look out for can help you feel better.

Does Bleeding Mean A Miscarriage

Bleeding during pregnancy

Vaginal bleeding when pregnant doesnt automatically mean youll experience a miscarriage.

In most incidences, pregnancy loss occurs between week five of pregnancy and week eight of pregnancy.

In most cases, in the first trimester of pregnancy, bleeding might last only about three days.

Although most women have one experience of bleeding, a small number will bleed throughout the entire pregnancy.

It can take the form of spotting, streaking, or period-like bleed, with or without clots.

When Should I Be Worried About Bleeding During Pregnancy

All vaginal blood loss during pregnancy needs medical assessment, even if its normal bleeding in the first trimester.

If you have any cramping, regular period-like pain, or fever, you should seek medical assistance quickly.

You especially need to see a doctor or midwife within 72 hours of any bleeding if you have a rhesus negative blood group .

This is to check whether your babys blood might have mixed with yours. If the blood has been mixed, and your baby is rhesus positive your body will see this as foreign matter and produce antibodies.

In the future, if you have any subsequent pregnancies, these antibodies will attack a baby with a positive blood type.

Spotting Throughout The Trimesters

In general, doctors dont tend to panic about light bleeding in the first trimester, when spotting is the most common. If spotting happens early in pregnancy, we dont get too hot and bothered about it, Alyssa Dweck, M.D., a gynecologist in Westchester County, New York, assistant clinical professor of obstetrics and gynecology at Mount Sinai School of Medicine, and coauthor of The Complete A to Z For Your V, tells SELF.

But if youre bleeding during the second half of your pregnancy, that could indicate a placental issue like placenta previa, says Dr. Dweck. Placenta previa is a condition where the placenta, the structure that provides oxygen and nutrition to the baby during pregnancy, implants over the cervix, the outlet for the uterus, the Mayo Clinic says. Placenta previa can be full or partial.

Thats a problem since the cervix is the passageway for your baby during birth. Any irritation to the cervix can lead to heavy bleeding and could be cause for emergency, says Dr. Dweck. If you have placenta previa, your doctor may recommend avoiding certain activities including sex, using tampons, or doing anything that could increase your risk of bleeding such as jogging, doing squats, and jumping, according to the Mayo Clinic.

What Causes Abdominal Pain In Early Pregnancy

Pain is very common in early pregnancy. The commonest reasons are outlined below.

Growing pains

You may experience aching and cramping pains as the womb increases in size in early pregnancy. This is common and may be eased with paracetamol which is safe for most women at any stage of pregnancy.

Muscle and joint pain

Muscle and pelvic joint pains are common especially towards the end of the first three months of pregnancy. This is because the joints of your pelvis are under the influence of pregnancy hormones. Simple pain relief and keeping active should help relieve this.

Constipation

Constipation and wind are common in early pregnancy. Your bowels work slower and you may find you open your bowels less frequently. It is important to drink plenty of fluids and eat a healthy diet with fresh fruit and vegetables every day. Peppermint water or tea may help relieve discomfort. Occasionally you may need a short course of laxatives from your GP.

Urine infection

Up to one in three women experience a urine infection during pregnancy. You may have a urine sample tested to check for this. To reduce the risk of getting a urine infection it is important to drink plenty of fluid and to completely empty your bladder when you go to the toilet. It may also be useful to pass urine after having sex. If your test shows that you have a urine infection you will be prescribed a short course of antibiotics.

What Other Problems Cause Bleeding During Early Pregnancy

Changes in the cervix: The normal hormone production during pregnancy can cause changes to the cervix, rendering it softer and more prone to bleeding. In addition, a cervical polyp may form, and this may bleed more easily during pregnancy. In both cases, spotting or light bleeding may be provoked following sexual intercourse or a pelvic examination.

Infection: A vaginal infection may cause spontaneous vaginal bleeding during pregnancy. The bleeding may be accompanied by an abnormal vaginal discharge.

Vaginal bleeding during early pregnancy may sometimes indicate a serious problem. Serious causes of bleeding in pregnancy include:

Miscarriage

  • Bleeding, abdominal pain, and back pain are common signs of miscarriage.
  • An exam shows that the cervix is open.
  • Tissue may be extruded through the cervix and vagina.
  • A miscarriage occurs in an estimated 15% to 20% of pregnancies, usually during the first 12 weeks of gestation.
  • A genetic defect confined to the specific embryo in question represents the most common cause of miscarriage.
  • Few circumstances exist wherein a miscarriage in progress can be prevented.

Vaginal Bleeding During Late Pregnancy

, MD, Wake Forest School of Medicine

The most common cause of bleeding during late pregnancy is

  • The start of labor

Usually, labor starts with a small discharge of blood mixed with mucus from the vagina. This discharge, called the bloody show, occurs when small veins are torn as the cervix begins to open , enabling the fetus to pass through the vagina. The amount of blood in the discharge is small.

In placental abruption, the placenta detaches from the uterus too soon. What causes this detachment is unclear, but it may occur because blood flow to the placenta is inadequate. Sometimes the placenta detaches after an injury, as may occur in a car crash. Bleeding may be more severe than it appears because some or most of the blood may be trapped behind the placenta and thus not be visible. Placental abruption is the most common life-threatening cause of bleeding during late pregnancy, accounting for about 30% of cases. Placental abruption may occur at any time but is most common during the 3rd trimester.

Bleeding may also result from disorders unrelated to pregnancy.

Uterine Fibroids Or Polyps

Uterine fibroids are growths of the uterus. They are not cancerous and may not cause symptoms.

If symptoms do occur, they may include:

  • Abnormal vaginal bleeding like spotting
  • Lower back pain that persists
  • Pelvic pain
  • Pain during sex
  • Urinary issues

Uterine polyps are growths in the lining of the uterus that are usually noncancerous . They may also cause spotting.

When To Call Your Doctor When You’re Spotting Or Bleeding During Pregnancy

No matter when it occurs, even if you have less than a dime-size spot of blood, call your doctors or midwife immediately to be safe, as any vaginal bleeding during pregnancy can be a symptom of a larger problem. Be prepared to answer detailed questions about the amount of blood you’ve lost and a description of how you’re feeling overall, advises Laura Riley, M.D., author of Pregnancy: The Ultimate Week-by-Week Pregnancy Guide. Your practitioner will decide whether there’s a cause for concern. If this is the case, she may want to monitor the situation by having you stay off your feet for a few hours and call back, or have you to come in immediately. She may also order an ultrasound to see what’s going on with the fetus. Dr. Riley says you should insist on being seen if you have any vaginal bleeding that makes you feel faint or soaks through a sanitary pad. You should also be seen if the bleeding is persistent or accompanied by pain or a fever.

Tests For Bleeding Problems During Later Pregnancy

Both placenta previa and placental abruption can cause heavy bleeding of bright red blood from the vagina.A vaginal examination is often used to help diagnose placental abruption, but could trigger heavier bleeding in the case of placenta previa. Therefore, an ultrasound scan should always be taken first, and digital vaginal examinations should be strictly avoided in the case of placenta previa.Tests used to diagnose placenta previa include:

  • Medical history
  • Ultrasound scan
  • Feeling the mothers belly to establish the babys position
  • Very gentle speculum vaginal examination .

Having ruled out placenta previa using the tests above, a digital vaginal examination may be used to identify placental abruption.

What To Do If You Get Further Abdominal Pain Or Vaginal Bleeding

Is Spotting A Symptom Of Pregnancy

Most women who have had a scan confirming an early pregnancy with a heart beat do not need a further scan in the early pregnancy unit. It is understandable to be concerned if you get more symptoms, however, in most women there is nothing to worry about.

If you get mild or moderate bleeding wear a pad and monitor the loss over the course of two days. In most cases the bleeding settles. If it increases in its amount during this time or persists as fresh bright red bleeding after two days then contact the early pregnancy unit for advice.

If you have heavy bleeding or bleeding with clots please contact the early pregnancy unit for advice.

If you get mild or moderate abdominal pain take pain relief and have a bath. Usually this helps relieve pain. If this does not help then either contact your GP or the early pregnancy unit if you are already under their care. We do not accept self-referrals.
