A tubal ligation is typically performed immediately after a woman has given birth. Irregular periods may occur after having a tubal ligation.

How can a tubal ligated woman can still be pregnant?

Since tubal ligation is considered a permanent form of birth control , it does require a minimally invasive surgical procedure to reverse it, so you can try to become pregnant. This involves reconnecting the blocked segments of the fallopian tubes to the remainder of the fallopian tubes.

What Happens During The Procedure

Just before open abdominal surgery, youll be given general anesthesia. The surgeon will make an incision a few inches long on your lower abdomen. The fallopian tubes can be seen and removed from this incision. Then, the opening will be closed with stitches or staples.

Laparoscopic surgery is a less invasive procedure. It may be performed under general or local anesthesia.

A tiny incision will be made in your lower abdomen. A laparoscope is a long tool with a light and camera on the end. It will be inserted into the incision. Your abdomen will be inflated with gas. This allows your surgeon to get a clear view of your pelvic organs on a computer screen.

Then a few additional incisions will be made. Theyll be used to insert other tools to remove the fallopian tubes. These incisions will likely be less than half an inch long. Once the tubes are out, the small incisions will be closed.

How Long Does It Take To Get Pregnant After Reversal

Many of our patients ask, How long does it take to become pregnant after a tubal reversal?

The monthly chance of becoming pregnant after tubal reversal is approximately 5 to 10%. Since tubal ligation reversal provides a chance each month, the cumulative chance of pregnancy increases over time and surpasses the pregnancy rate associated with treatment by IVF.

Most of our patients become pregnant within 12 months of having tubal ligation reversal at our center, but for some patients pregnancy may not occur until one or more years after their reversal procedure.

How Is Tubal Reversal Done

Is it possible to get pregnant after a tubal ligation?

You’ll need to go to a hospital or an “outpatient” center — a place where you don’t stay overnight after surgery. You’ll be given general anesthesia, which means you’ll be pain-free and won’t be awake during the operation.

Your surgeon places a small lighted scope, called a laparoscope, through your belly button and into the pelvis area. This lets them look at your fallopian tubes and decide if reversal surgery is possible.

If they decide it’s OK to do the reversal, your doctor then makes a small surgical cut, called a “bikini cut,” near your pubic hair line. Microscopic instruments attached to the end of the laparoscope let them remove any clips or rings that were used to block your tubes, and reconnect the ends of the tubes to the uterus, using very small stitches.

The surgery usually takes about 2 to 3 hours.

Ask your doctor about other approaches to a tubal reversal.

Having A Child After Hysterectomy

If you want to have children but you need a hysterectomy for medical reasons, it is possible for you to start a family. While you might be able to use your own eggs, you cant carry the pregnancy yourself.

One option is to have your eggs harvested for future fertilization and surrogate implantation. Harvesting can be done before the surgery if your ovaries will be removed, or after surgery if your ovaries are to remain intact. While a surrogate will carry the child, it will be your biological child.

If egg harvesting is not possible or you do not wish to go that route, there are other options. A male partner may provide sperm for a surrogate pregnancy, either of a donor egg or the surrogates egg. A female partner may carry a pregnancy resulting from her egg being fertilized. You may also consider adopting a child as an alternative.

What Are The Alternatives To Reversal

If youve had a tubal ligation, in vitro fertilization is your other option for getting pregnant. In IVF, your eggs are mixed with your partners sperm in a lab. Fertilized eggs are then implanted directly into your uterus and pregnancy can proceed as usual from there if the procedure is successful.

The choice to have a tubal ligation reversal or undergo IVF is different for every woman. However, there is evidence that IVF might be a better choice than tubal ligation reversal for women over 40, while the opposite is true for women under 40.

One concern is cost. For women under 40, reversal is often less expensive, including the costs of pregnancy afterward. IVF is usually less expensive for women over 40.

Another factor to consider is the likelihood of pregnancy after each procedure. For example, the pregnancy rate for women over 40 who have a tubal reversal is half the rate of those who have IVF. For women under 35, pregnancy after a reversal is almost twice as likely than from IVF. For women ages 35 to 40, pregnancy is also almost twice as likely with tubal reversal than with IVF.

Women who have had tubal ligations sometimes regret their decision and desire fertility in the future. There are 2 options for fertility after tubal ligation, tubal reversal surgery and in vitro fertilization IVF. Both of these are reasonable options and how the woman chooses to proceed should be based on an educated consideration of the pros and cons of each.

Most women have their tubes tied before they leave the hospital after they deliver a baby, or they come in for tubal ligation by laparoscopy which is an outpatient surgical procedure done through a scope that goes through the belly button.

Tubal reversal surgery usually requires a laparotomy which is a much larger incision on the abdomen usually approximately 4 to 6 inches in length. Since the skin and all the muscles and other tissues of the abdominal wall must be cut through there is considerably more discomfort and a much longer recovery time following the surgery as compared to a laparoscopic surgery such as with a tubal ligation.

Some surgeons are now performing tubal reversal surgeries through the laparoscope. However, this is a relatively difficult technique and there is not yet sufficient published data to know what pregnancy rates will be after laparoscopic tubal reversal.

Tubal Reversal Success And Increased Risks

Whether or not tubal reversal surgery can be successfully performed depends on two factors. Is there enough healthy tissue on each end of the tube to be reconnected, explains Dr. Bromer, and is the reconnected tube long enough to function properly? Unfortunately, not all patients have tubes that can be reconnected.

Sometimes even when reattaching the tubes is successful, there can be scarring of the tissue that interrupts the flow of the tube, says Dr. Bromer. This happens in about 20 percent of the cases. If, however, the HSG confirms that the tubes are open, then the patient can begin trying to get pregnant on her next cycle.

There is also an increased risk for ectopic pregnancy after tubal reversal surgery. One risk with tubal reversals is that there is a 10 to 20 percent chance of having an ectopic pregnancy after tubal reversal surgery, says Dr. Bromer. Ectopic pregnancy occurs when a fertilized egg gets stuck in the Fallopian tubes and implants there. Ectopic pregnancies must be terminated with medications or surgery to avoid a rupture of the Fallopian tube, which can be life threatening.

Since the tubes cannot always be reconnected, the success rate for achieving pregnancy after a tubal reversal is around 40 percent for women under 37.

Common Questions About Tubal Ligation Reversal

Tubal ligation reversal might sound like a scary process, but its actually a safe, straightforward procedure for healthy women, and its a fairly common fertility treatment option for restoring fertility.

The problem of infertility is something many women face today most just dont realize how common it is or that there are fertility treatment options available. Its estimated that around 44% of women have sought medical treatment for infertility, and as many as 90% of infertility cases can be treated effectively through drug therapy or surgery.

Before choosing to have a tubal ligation reversal, you probably want some questions answered. While talking to a medical professional before making any decisions is always advised, heres a quick look at some of the most common questions women ask when theyre considering a tubal reversal for restoring fertility:

How big will the incision be? The incision in the procedure is usually around two inches long and will be in the middle of your lower abdomen, usually around the bikini line. Its really not a big incision at all, and it heals quickly, around six to eight weeks in most cases.

How long does it take to get pregnant after the surgery? This depends on each individual, but most patients find that they can get pregnant within six months after the surgery. In some cases, it can take as long as a year. The success rates for tubal ligation reversal procedures are as high as 65% when the time span is at one year.
