Nausea And Vomiting Of Pregnancy

HOWARD ERNEST HERRELL, MD, East Tennessee State University, Johnson City, Tennessee

Am Fam Physician. 2014 Jun 15 89:965-970.

Nausea and vomiting of pregnancy affects nearly 75% of pregnant women. The exact cause is unknown. In most cases, it is a mild, self-limited condition that can be controlled with conservative measures and has no adverse fetal sequelae. About 1% of women develop hyper-emesis gravidarum, which may result in adverse outcomes for the mother and fetus. Patients with nausea and vomiting of pregnancy should be evaluated for other causes, particularly if symptoms are unremitting or presentation is atypical. Initial treatment is conservative and includes dietary changes, emotional support, and vitamin B6 supplementation. Several safe and effective pharmacologic therapies are available for women who do not improve with initial treatment. Women with hyperemesis gravidarum may require more aggressive interventions, including hospitalization, rehydration therapy, and parenteral nutrition.

Nausea and vomiting occur in up to 74% of pregnant women, and 50% experience vomiting alone.1,2 Although the term morning sickness is commonly used to describe nausea and vomiting of pregnancy, the timing, severity, and duration of symptoms vary widely. Approximately 80% of women report that their symptoms last all day, whereas only 1.8% report symptoms that occur solely in the morning.2

What Does Pregnancy Nausea Feel Like

How pregnancy nausea feels varies among women, and can even feel different to the same woman at different times of the day.

Pregnancy nausea or morning sickness can feel like:

  • A general, lingering sense of feeling like you need to vomit, but without the urgency to do so.
  • A sudden, urgent need to vomit.
  • A feeling that youre hungry, but without the ability to stomach anything.
  • Dizziness or weakness.

Some women experience nausea during pregnancy and never actually vomit, while others vomit several times a day. The vomiting can sometimes provide a reprieve from nausea, until it returns.

Pregnancy nausea is often exacerbated by the intense fatigue that is also experienced during the first trimester.

My first pregnancy I felt nauseous in the morning and my second it felt like motion sickness up until the second trimester.

Editors Note:

Exhaustion and tiredness can even cause nausea in a person who isnt pregnant. So, the fact that women in their first trimester have a double whammy of both exhaustion and nausea can make them compound one another.

Eat Small Meals Instead Of Big Meals

Break up the portions and timing of your meals. Hormones produced during pregnancy, especially in the first trimester, slow down your digestive system muscles making it harder to digest larger meals. Eat smaller portions and eat more frequently throughout the day. Having an empty stomach or low blood sugar can cause nausea or make nausea worse. Eating small meals every few hours can keep your blood sugar steady and your stomach feeling full, but not too full. If youre battling morning sickness, also stay away from spicy foods. Youll want to eat foods you know are easy to digest.

What Causes Morning Sickness

Hyperemesis Gravidarum | Pregnant women suffering from condition far worse than morning sickness

The exact cause of nausea and vomiting in pregnancy is not clear. Most evidence points to rapid changes in hormone levels. These fluctuations may cause changes in the muscle contraction and relaxation patterns of your stomach and intestines, thus leading to nausea and vomiting.

The hormones that seem to have the most to do with this process include the pregnancy hormone human chorionic gonadotropin , estrogen, and progesterone. Abnormal levels of thyroid hormones have also been reported in women with severe vomiting, although a cause-and-effect relationship remains unclear. Some studies have shown that nausea is worse when your blood sugar level is low. Some researchers have found that women who are more likely to have nausea from birth control pills, migraines, or motion sickness are at higher risk for nausea and vomiting in pregnancy.

If you have a family history of hyperemesis gravidarum, you are more likely to have the condition. What causes hyperemesis gravidarum remains unknown despite active research.

The more popular theories fall into three areas:

  • Gastrointestinal:Helicobacter pylori bacteria that live in the intestinal tract may cause the development of peptic ulcer disease. These bacteria are found in a greater percentage among pregnant women and greater still in women with hyperemesis gravidarum. Antibiotics are used to treat all of these conditions.

Pregnancy And Nausea: Which Medications Are Safe And Effective To Use

    Medicine For Vomiting In Pregnancy

It may come as no surprise that up to 80% of women experience nausea during pregnancy, especially in the early stages. Severe nausea and vomiting is a reason for higher rates of pregnancy termination. So, it follows that the early, effective, and safe treatment of nausea is crucial for both the mothers and the infants health. Because medications are prescribed to manage vomiting and nausea in the first trimester , pregnant women and clinicians often have concerns about the potential effects of anti-nausea or antiemetic drugs.

Malformations at birth are exceptionally rare, so finding the cause and effect of a single drug and resulting abnormality is extremely difficult. A recent study of 1.8 million pregnancies showed a direct correlation between the use of specific anti-nausea medications with a risk of birth abnormalities. Due to these concerns, Health Canada recommends NOT to use ondansetron for managing nausea during pregnancy. Pharmacogenetics, the study of how a persons genetics and medications interact, can check whether commonly used anti-nausea drugs will be effective for an individuals metabolism. Drugs such as dolasetron, ondansetron, palonosetron, and tropisetron can have varied effectiveness from person to person based on genetics. Pharmacogenetics may also help to understand the risks associated with these drugs.

    Keep Your Mouth Squeaky Clean

Regular brushing helps keep your mouth fresh and reduces queasiness in the future. It also decreases the risk of damage to teeth from vomiting.

Brush your teeth or rinse your mouth after each meal and after bouts of vomiting once your tummy has settled a bit. If your usual toothpaste contributes to nausea , ask your dentist to recommend another option or a good rinse. Or use plain water.

When Are Medicines For Nausea And Vomiting Prescribed

Medicines for nausea are usually only prescribed once your doctor is sure what is causing your feeling of sickness . This is because, in a lot of cases, if you treat the condition causing your nausea, it will go away. For example, if your nausea is caused by taking too much of a particular medicine, your doctor will reduce the dose and your nausea will often go away. If the cause of your nausea is not known, your doctor can still prescribe a medicine for you but it is much better to know what the cause is so the right medicine can be chosen.

Anyone who’s ever dealt with morning sickness will tell you: It’s no joke. And now medical experts are shedding a bit more light on the importance of adequately treating pregnancy-induced nausea.

According to Britain’s Royal College of Obstetricians and Gynecologists, four out of five pregnant women experience morning sickness through pregnancy and, despite its name, this affliction can strike all day, every day. Because of this, the group has rolled out new guidelines stressing the importance of proper treatment for morning sickness.

“Women with persistent nausea can often feel that there is a lack of understanding of their condition,” Dr. Manjeet Shehmar, the lead author of the guidelines, said, according to the BBC. “They may be unable to eat healthily, have to take time off work, and feel a sense of grief or loss for what they perceive to be a normal pregnancy. It is therefore vital that women with this condition are given the right information and support and are made aware of the therapeutic and alternative therapies available to help them cope.”

Things To Consider When Buying Medicine For Nausea

Nausea medicine comes in many different forms of products and formulations, and you should know that not all of these will work well for your case. Understanding these specifics before making a purchase will help you land the best anti nausea medication that will provide you real and instant relief.

    • Different Causes. Nausea can be caused by a multitude of different factors. Some of these include eating a bad meal, dizziness caused by motion, pregnancy, and viral infections. Some of these causes can be remedied by a product that directly acts on the nausea itself, such as those formulated for motion sickness and stomach flu.

However, because some cases of nausea are brought about by different underlying factors, nausea medicine will only work to temporarily relieve the problem. For instance, taking anti nausea medication to relieve nausea caused by a viral infection will only provide temporary relief against the symptoms, and not remedy the cause all together. Understanding what has caused your nausea will make it easier for you to choose a product that will work well for your specific case.

    • Homeopathic Versus Synthetic. There are many herbal homeopathic remedies in the market that aim to provide relief against nausea, and for some people, these options may be more than enough to provide lasting results.
